Emergency Cards

In the event of illness or injury it is imperative that we are able to conform to the wishes of parents or guardians regarding the release of their child for treatment.  It is MANDATORY that EMERGENCY CARDS be completed and signed for each student.  Every attempt will be made to contact the parent or guardian before calling the persons listed on the card/form as emergency contacts.  Your child will be released ONLY to the person listed on the emergency card/form.

If your child does not have an emergency card/form on file, or if you have changed some of your information, please contact the main office or health office to give updated information.

 

Illness

 

Please do not send your child to school if he/she exhibits any of the following symptoms:

  • Fever- 100 degrees Fahrenheit or greater
  • Cold symptoms-runny/stuffy nose, cough, body aches, chills
  • Undiagnosed Rash
  • Vomiting/diarrhea
  • Swollen glands/sore throat

Your child must be fever free for 24 hours before returning to school without using fever-reducing medicines, such as acetaminophen or ibuprofen

Contagious illnesses such as:  Strep Throat, Chicken Pox, Coxsackie, Conjunctivitis, Fifth’s Disease must be reported to the nurse

The nurse must be notified if your child received: stitches, a fracture or burn

If a student needs crutches a doctor’s note must be given to the nurse

Good hand hygiene is extremely important!!  Please have your child cover mouth and nose when coughing or sneezing.  Please remember to cough into “elbow” to keep germs off hands.  Prevention with good hygiene is essential!!
